About the Role
We are seeking an experienced and compassionate
Case Manager RN to join our remote team. In this role, you will assess, plan, and coordinate care for patients, ensuring they receive optimal healthcare services. As a remote case manager, you will work closely with patients, families, and healthcare providers to deliver personalized and efficient care plans tailored to individual needs.
Key Responsibilities
- Conduct comprehensive assessments of patients' healthcare needs.

- Coordinate and monitor services, ensuring compliance with care plans.

Qualifications
- Active and unrestricted RN license in Michigan.
