We are looking for a compassionate and organized Care Coordinator to join our healthcare team. As a Care Coordinator, you will work closely with patients, families, and healthcare providers to ensure seamless care delivery. Your role will involve managing patient care plans, facilitating communication across the care team, and advocating for patients’ needs to improve health outcomes.

• Coordinate and manage patient care from initial consultation through ongoing treatment
• Communicate with patients, healthcare providers, and other stakeholders to ensure appropriate care delivery
• Develop, update, and track care plans based on individual patient needs
• Facilitate referrals to specialists and follow up to ensure that patients attend appointments
• Assist patients in navigating the healthcare system, including providing resources and support
• Monitor patient progress and ensure any necessary changes are made to care plans
• Work with insurance companies to ensure proper coverage and documentation
• Support patient education, helping them understand their condition, treatment options, and care instructions
• Provide emotional and logistical support to patients and their families throughout their care journey

• Associate’s or Bachelor’s degree in Healthcare, Nursing, Social Work, or a related field
• Previous experience in healthcare or care coordination preferred
• Strong knowledge of healthcare systems and processes, including patient care and insurance systems
• Excellent communication and interpersonal skills, with a customer-focused mindset
• Proficiency with electronic health records (EHR) and care coordination software
• Ability to work independently and handle a high volume of tasks
• Compassionate, patient, and resourceful, with a strong commitment to improving patient outcomes
• Must be located in the Central Time Zone or willing to work within Central Time Zone hours
